In this dynamic role, you'll conduct telephonic and/or in‑person assessments in a variety of settings-members' homes, hospitals, behavioral health facilities, and long‑term care environments. You will be the trusted clinical partner who develops personalized, cost‑effective care plans centered around each member's unique needs, goals, and preferences.
As a Care Manager, you will:
- Build strong relationships with members, caregivers, and providers.
- Support safe and seamless transitions of care-from hospital to home, or from facility to independent living.
- Address not only clinical needs, but also behavioral, emotional, and social determinants of health such as housing stability, food access, and caregiver support.
- Monitor progress and evaluate care plan effectiveness, adjusting as member needs evolve.
- Collaborate within an interdisciplinary team to ensure timely, coordinated services.
- Document your work accurately and efficiently within the electronic health record.
Required Qualifications
- Resident of New Jersey - Burlington/Ocean County and South
- BSN RN with an active unrestricted license in New Jersey
- Must possess reliable transportation and be willing and able to travel up to 75%of the time. Mileage is reimbursed per our company expense reimbursement policy. Face to Face/Telephonic visits are determined by the members needs/wants.
- 3 or more years clinical nursing experience in pediatrics, special needs and or adult population.
Preferred Qualifications
- Prior case management experience in an integrated care model.
- Bilingual skills.
- Knowledge of medical terminology.
- Strong digital literacy.
- Ability to communicate professionally and compassionately with members and community partners.
- Demonstrated ability to maintain confidentiality and handle sensitive information responsibly.
- CCM certification (Certified Case Manager).
- Previous remote work experience.
- Prior case management experience in an integrated care model.
- Bilingual skills.

Pay Range
The typical pay range for this role is:
$66,575.00 - $142,576.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
