A Brief Overview
Serves as a clinical specialist responsible for comprehensive assessment, planning, implementation, and evaluation to engage and educate members toward optimal health outcomes. Uses clinical tools and data analysis to determine member needs and benefit eligibility, ensuring coordination of appropriate programs and services. Identifies and communicates opportunities to improve medical appropriateness and enhance the quality of care.
What you will do
• Applies clinical criteria, evidence‑based guidelines, organizational policies, and regulatory standards to generate actionable insights that promote appropriate utilization of healthcare services and alignment with care management programs.
• Provides member coaching to support adoption of healthy lifestyle behaviors, offering education on chronic conditions and empowering members to make informed decisions about their health.
• Develops, implements, and continuously monitors individualized care plans in partnership with members, providers, and interdisciplinary teams to support optimal health outcomes.
• Identifies barriers to care and provides education and resources to help members overcome obstacles and stay on track with goals and physician prescribed recommendations.
• Recognizes and escalates quality‑of‑care concerns through established clinical and organizational pathways.
• Delivers services in adherence to regulatory, accreditation, and organizational standards.
• Plans, schedules, and leads onsite wellness engagement activities and health‑focused educational events to promote preventive care and member well‑being.

Position Overview
We are seeking an Onsite Health Coach (RN) to join our team and provide high‑quality, evidence‑based health coaching to employees in their workplace setting. In this role, you will assess individual health needs, develop personalized action plans, and deliver continuous support and education to help members achieve sustainable health and wellness goals.
This is an onsite position based in Winston‑Salem, North Carolina. It is not a remote role. Training will occur remotely over six to eight weeks, after which the Health Coach will be required to work onsite. You will be supported by both Aetna leadership and the client's onsite health promotion team.
Role Summary
The Health Coach Consultant (RN) uses a collaborative, member‑centered approach-assessment, planning, implementation, and evaluation-to engage, educate, and empower members in achieving and maintaining optimal health. This role supports preventive care, lifestyle modification, and early intervention through personalized coaching and outreach.

Primary Responsibilities:
- Performs strategic outreach to identified high risk members, including in-person onsite activities and opportunities for creative engagement
- Meets one-on-one to provide motivation, monitor progress, and assist employees in developing personal and measurable health goals and plans
- Reduces gaps in member health service by providing referrals
- Supports the Client Company's Culture of Health workplace initiatives
- Promotes strategies for lifelong wellness in the healthy member population through education and accountability for prevention and lifestyle interventions focusing on nutrition, physical activity, weight management, stress management
- Assesses members using clinical tools and information/data review, conducts comprehensive evaluation of member's needs and benefit plan eligibility for available integrated internal and external programs/services.
- Utilizes assessment techniques to determine member's level of health literacy, technology capabilities, and/or readiness to change.
- Applies and/or interprets applicable criteria and guidelines, health/wellness management plans, policies, procedures, regulatory standards while assessing benefits and/or member's needs to enable appropriate utilization of services and/or administration and integration with available internal/external programs.
- Utilizes influencing/motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behavior changes to achieve optimum level of health.
- Provides coaching, information, and support to empower the member to make ongoing independent medical and/or healthy lifestyle choices.
- Interprets and utilizes clinical guidelines/criteria to positively impact members health Provides up-to-date healthcare information to help facilitate the member's understanding of his/her health status.
- Helps member actively and knowledgably participate with their provider in healthcare decision-making.
- Monitors established plans of care
Work Schedule
- Five days per week onsite in Winston‑Salem, NC
- Monday-Friday, 8am-4:30pm with some flexibility based on client needs
- Occasional travel to other client locations may be required, including potential overnight travel
- No weekends
- Company‑designated holidays off
- Schedule adjustments may occur according to plan sponsor requirements
Required Qualifications
- 3+ years of clinical experience as an RN in acute care setting (med/surg; ER)
- Must have an active, current, and unrestricted compact RN licensure in the state of North Carolina
- Minimum of 1 year of previous coaching experience
- Ability to commute to/from Winston-Salem, North Carolina for onsite work each shift; Monday through Friday 8am to 4:30pm EST.
- Must be willing and able to work outside of standard hours Monday through Friday due to business needs
- Must live within 45 minutes of Winston-Salem, North Carolina
Preferred Qualifications
- Direct education and coaching experience with individuals for disease prevention, health education, or behavior change preferably in a corporate setting
- Managed care experience
- Experience in multiple areas of nursing in an acute setting
- Motivational Interviewing experience
- A certification as a health coach from a recognized organization
- Excellent communication, interpersonal, and motivational skills
- Able to work independently and as part of a team
- A passion for helping others improve their health and well-being
- Proficient in Microsoft Office and online platforms
Education
- Associate degree in nursing required
- BSN or a bachelor's degree in health education or health promotion preferred

Pay Range
The typical pay range for this role is:
$60,522.00 - $129,615.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
